Groups are encouraged to fill out application forms on the computer and email a file of the completed form to admin@carmelgreenteen.org.

All groups are required to provide an electronic or hard-copy form of the application and supporting documents by the application due date. All original signature verification pages must be turned in at the grant review session or mailed to the address below so that they are in our hands on the grant review date:
